We’re excited you’re considering The Dog Den or The Puppy Den for your dog.

This enrollment form helps us get to know your dog as an individual — their history, preferences, routines, and needs — so we can provide care that is safe, intentional, and supportive. There are no “right” or “wrong” answers here. The more you’re able to share, the better we can understand your dog and communicate with you clearly about next steps.

Our goal is to make sure this environment is a good fit for your dog’s well-being and that you feel confident and comfortable with the care we provide.

Health Records Required

To complete enrollment, we’ll need copies of your dog’s most recent health records, including:

• Bordetella vaccine

• Distemper (DHLPP) vaccine

• Rabies vaccine (if over 16 weeks old)

• Clear fecal test results (within 6 months of start date)

• Spay/neuter information (for dogs over 6 months)

You can upload these documents directly on the enrollment form, or email them to info@thedogden.com.

What Happens Next

Once we receive your completed enrollment form and health documentation, our team will review everything and follow up by email within 1–2 business days to discuss next steps.
